REQUIREMENTS

1. Medical Certificate
2. Pinakabagong reseta ng gamot na nakasulat ang petsa ng issuance, pirma, at lisensya ng doktor
3. Certificate of Indigency mula sa Barangay o Municipal/City Social Worker (kung available)
4. Valid IDs ng informant at pasyente

PROSESO NG PAG-APPLY

1. Pagsumite ng requirements sa opisina ng MSSD
2. Screening at verification ng mga papeles
3. Interview
4. Release ng endorsement
5. Pagsumite ng endorsement sa itinalagang partner pharmacy
6. Release ng gamot mula sa pharmacy
